Job summary

Trilogy Health Services is seeking a Physical Therapist (PT) to provide comprehensive PT services, including assessment, treatment planning and therapeutic interventions in an interdisciplinary setting. Must hold a current state PT license and CPR is preferred; 0-1 years experience is acceptable.

Responsibilities include conducting assessments, developing tailored treatment plans, educating patients and families, and maintaining compliant documentation while collaborating with the care team.

Qualifications

• Must have and maintain a current, valid state PT licensure
• Current CPR certification, preferred
• 0-1 years of experience in physical therapy
• Licensed PT must adhere to professional standards and ethics

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities
• Educate patients and families on exercise techniques, posture, body mechanics and self-management
• Maintain accurate documentation of patient progress and treatment plans per policy
• Collaborate with interdisciplinary team to ensure comprehensive patient care
• Perform other duties as assigned

Education

Degree in Physical Therapy from an accredited program
