Job summary

Trilogy Health Services is seeking a Physical Therapist to provide a full range of PT services in an interdisciplinary setting. The position requires a Master’s degree in Physical Therapy and current state licensure, with travel as needed.

Responsibilities include assessing patients, planning and implementing treatment, educating families, and maintaining thorough documentation. Collaboration with the care team is essential to deliver high-quality rehab services.

Qualifications

• Conduct thorough assessments to evaluate patients' physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans including therapeutic exercises and modalities.
• Educate patients and families on exercises, posture, and strategies to manage conditions.
• Maintain accurate documentation of progress and outcomes in compliance with policies.

Responsibilities

• Conduct thorough assessments to evaluate patients' physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients' physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
