Job description

BAYADA Home Health Care is currently seeking an experienced Weekend Physical Therapist (PT) for home health visits throughout Berks County. This position services adult and geriatric clients on a per-visit basis.

Requirements include at least one year of clinical experience as a licensed Physical Therapist. As a home care PT, you will be a key member of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities include:

• Conduct home visits within designated geographic areas.
• Perform diagnostic tests such as mobility, strength, balance, and safety assessments.
• Implement wound prevention and repair interventions using evidence-based dressings and topical agents.
• Develop and update individualized care plans, including manual therapy, gait training, and balance exercises.
• Assess and revise care plans regularly, collaborating with nursing staff as needed.
• Educate clients and their families on rehabilitative care and activities promoting health and independence.
• Document observations and interventions accurately using electronic tablets.

Qualifications:

• Current Pennsylvania Physical Therapist (PT) license.
• Minimum of one year recent experience as a PT.
• Graduation from an APTA-approved program, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; prior EMR experience preferred.
