We have a Per Diem Registered Nurse opportunity available now, doing weekend Home Health visits in Senior Living communities throughout Chester County.

Must have weekend availability between the hours 8:30am - 5pm.

Prior Home Health And Oasis Experience Preferred.

Responsibilities:

• Follow care plans according to patient needs
• Perform home visits and administer medication, wound care, treatments, and procedures
• Monitor client conditions and report changes
• Document care and follow doctors' orders
• Conduct client assessments and coordinate case management

Qualifications:

• Current Pennsylvania RN license
• Minimum one year of recent clinical experience
• Home care experience preferred
• Graduate from an accredited nursing program
• Ability to work independently, manage time, and travel as needed
• Strong interpersonal and computer skills, EMR experience preferred
