Job description

BAYADA Home Health Care is currently seeking an experienced Weekend Physical Therapist, PT for an opportunity performing home health visits throughout Schuylkill County. This office services adult and geriatric clients on a per-visit basis.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, allowing them to receive the necessary medical care to remain at home.

Responsibilities for a PT - Physical Therapist include:

1. Make home visits to clients in designated geographic areas.
2. Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Implement protective interventions to prevent wounds and promote healing, utilizing evidence-based dressings and topical agents.
4. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
5. Assess and revise care plans regularly, collaborating with nursing in multidisciplinary team meetings as appropriate.
6. Educate clients, family members, or representatives on rehabilitative care and activities to promote health, safety, and independence.
7. Document observations, interventions, and evaluations accurately using a state-of-the-art touchpad tablet.

Qualifications for a PT - Physical Therapist include:

• A current Pennsylvania Physical Therapist (PT) license.
• At least one year of recent clinical experience as a PT.
• Graduation from an American Physical Therapy Association-approved program, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) is preferred.
