Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, with the goal of improving function and quality of life.

The role involves collaboration with facility staff, supervision of therapy assistants, and meticulous documentation to track progress and outcomes for each patient.

Qualifications

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress and updating care plans and goals

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
