Job summary

Enhance Rehab is seeking qualified Physical Therapists to join our team in a skilled nursing facility setting. You will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure the highest quality of care.

The role includes supervising Physical Therapist Assistants, re-evaluating progress, updating plans of care, and completing timely documentation such as evaluations, re-evaluations, discharges, and daily progress notes.

Qualifications

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Collaborate with facility staff to ensure excellent care for your patients
