Job summary

Enhance Rehabilitation is seeking qualified Physical Therapist candidates to join our dynamic team in a SNF setting. As a Physical Therapist, you will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure excellent patient care.

You will also educate patients and families about injuries, progress and outcomes, while supervising Physical Therapist Assistants and completing timely documentation to support continuity of care.

Qualifications

• Evaluating new patients and developing plans of care.
• Setting measurable, functional goals.
• Supervising Physical Therapist Assistants.
• Completing timely documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Collaborating with facility staff to ensure excellent care.

Responsibilities

• Providing excellent care for our patients and addressing their concerns.
• Educating patients on their injury, progress and probable outcomes.
• Evaluating and treating patients following an individualized plan of care.
• Supervising PTAs and ensuring quality therapy delivery.
• Re-evaluating patient progress and updating goals as needed.
