Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients, develop individualized plans of care, and work closely with facility staff to ensure quality outcomes.

The role includes supervising Physical Therapist Assistants, re-evaluating progress, updating plans of care, and maintaining timely documentation of evaluations, re-evaluations, discharges and daily progress notes.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
