Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a skilled nursing facility setting. You will evaluate and treat patients following an individualized plan of care to support recovery and functional goals.

In this role, you will supervise Physical Therapist Assistants, re-evaluate progress, document thoroughly, and communicate progress with patients and families to facilitate positive outcomes.

Qualifications

• PTs provide evaluation and treatment following an individualized plan of care.
• Must be licensed Physical Therapist in the state.
• Collaborate with facility staff to ensure high-quality patient care.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
