Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, while collaborating with facility staff to ensure excellent patient care.

The role emphasizes providing high-quality care, documentation, and educating patients about their injury, progress, and outcomes. Supervising Physical Therapist Assistants and ongoing care optimization are part of the duties.

Responsibilities

• Provide excellent care for patients.
• Provide excellent customer service to patients and their families.
• Collaborate with facility staff to ensure excellent patient care.
• Evaluate new patients, develop plans of care, and set measurable functional goals.
• Treat patients, address concerns, and educate them on injury, progress and outcomes.
• Supervise Physical Therapist Assistants.
• Re-evaluate patient progress, revise plans of care, and reset goals.
• Complete timely documentation: evaluations, re-evaluations, discharges, and daily progress notes.
