Reliant Rehabilitation is seeking a Physical Therapist to join our therapy team serving patients in skilled nursing facilities. The role emphasizes high-quality patient care, development of treatment plans, and collaboration with facilities to ensure compliant, effective therapy services.

Qualifications

• Graduate of an APTA accredited school of physical therapy.
• Valid physical therapist state license.
• Comfortable with clinical documentation and basic computer skills.

Responsibilities

• Provide physical therapy and related services to patients in SNFs as part of Reliant Rehabilitation.
• Develop initial and ongoing treatment plans and discharge plans.
• Supervise assistants and Rehab Techs and coordinate care with facilities.
• Maintain standards in accordance with state and federal regulations.
• Communicate effectively with patients, facilities, and team members.
