Reliant Rehabilitation is seeking a Physical Therapist to provide therapy services in SNF settings, evaluating patients and creating treatment plans while maintaining high professional standards.

You will collaborate with a multidisciplinary team, supervise assistants as needed, and adhere to regulatory requirements to deliver quality patient care and support our clinical ladder and advancement opportunities.

Qualifications

• Graduate from an APTA accredited school of physical therapy.
• Maintain current state licensure as a physical therapist.
• Strong clinical documentation and computer literacy required.

Responsibilities

• Provide physical therapy services with quality patient care.
• Develop initial and ongoing plans of care and discharge plans.
• Supervise PTAs and Rehab Techs when applicable.
• Ensure compliance with state and federal regulations.
