Job summary

Reliant Rehabilitation is seeking a Physical Therapist to provide therapy services in skilled nursing facilities. The role includes evaluating patients, developing and implementing treatment plans, and coordinating discharge planning, with supervision of support staff as needed.

New grads are welcome and there is potential for a sign-on bonus; PRN and full-time options may be available depending on location.

Qualifications

• Graduate of an APTA accredited school of physical therapy.
• Valid physical therapist state license.
• Computer literate and proficient in clinical documentation.
• Basic Internet navigation skills.

Responsibilities

• Provide physical therapy and related services for patients.
• Develop initial and ongoing treatment plans of care and discharge plans.
• Supervise assistants and Rehab Techs as applicable.
• Maintain professional conduct and comply with state and federal regulations.
• Be aware of various payment models related to billing and treatment guidelines.
