Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Tampa, FL. The role involves delivering high-quality PT services, evaluating residents, and coordinating with an interdisciplinary team to plan and implement individualized treatment plans.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development.

Responsibilities

• Provide physical therapy services per established standards and productivity targets.
• Document and bill therapy services according to procedures.
• Evaluate residents and develop treatment plans.
• Conduct regular screenings to determine intervention needs.
• Perform evaluations to guide treatment planning and implementation.
• Develop short- and long-term goals and methods.
• Communicate results to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
• Perform other duties as assigned.
