Job summary

Bayshore Residence and Rehabilitation Center in Duluth, MN, is seeking a motivated Physical Therapist (PT) to join our Rehabilitation Services team. The PT will evaluate, plan, and implement individualized therapy programs to maximize residents’ mobility and independence in a skilled nursing facility setting.

Responsibilities include direct therapy, documentation, care coordination with an interdisciplinary team, education for residents and families, and ongoing progress monitoring.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Minnesota PT license in good standing.
• Knowledge of federal, state, and local regulations related to skilled nursing facilities.
• Strong communication, teamwork, and organizational skills.

Responsibilities

• Evaluate residents’ functional abilities and develop individualized treatment plans.
• Provide direct physical therapy interventions including therapeutic exercise, gait training, balance activities, and pain management.
• Document evaluations, treatment sessions, progress notes, and discharge summaries in accordance with requirements.
• Collaborate with an interdisciplinary team to coordinate resident care.
• Educate residents, families, and caregivers on therapy programs and safety techniques.
• Monitor progress and adjust treatment plans as needed.
• Maintain HIPAA compliance and infection control protocols.
