Presbyterian Homes & Services (PHS) in Minnesota is seeking a Full Time Physical Therapist for Boutwells Landing. The role covers TCU/SNF, Assisted Living, Independent Living, Home Health, and Outpatient settings with a focus on resident quality of life and compassionate care.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Current licensure in the state(s) of practice as required.
• Demonstrated compatibility with PHS's mission and operating philosophies.
• Demonstrated ability to read, write, speak and understand English to communicate with all customers.

Responsibilities

• Conduct a comprehensive assessment & evaluation using subjective and objective measures.
• Develop treatment plans and programs in accordance with physician orders.
• Provide quality treatments, actively progressing goals to achieve highest quality of life.
• Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.
• Provide accurate and timely documentation & billing in electronic medical record.
• Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.
