Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Olivia, MN. This full-time role offers a $5,000 sign-on bonus plus tuition reimbursement and a choice of full-time, part-time, or PRN positions.

Responsibilities include providing therapy services, documenting and billing, evaluating residents, and creating treatment plans to achieve measurable goals while ensuring CMS compliance in NetHealth EMR.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• CMS documentation expectations in NetHealth EMR systems.

Responsibilities

• Provide physical therapy services to residents per established standards.
• Document and bill therapy services following procedures.
• Evaluate residents to plan and implement therapy programs.
• Develop treatment plans with short- and long-term goals.
• Communicate findings to residents, families, and the team.
• Complete compliant documentation in NetHealth EMR per CMS rules.
