Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Olivia, MN. This full-time role offers a $5,000 sign-on bonus plus tuition reimbursement, with additional Part Time or PRN options available.

You will plan, implement, and monitor physical therapy services, ensuring quality resident care and compliance with CMS and company procedures. Active licensure and ongoing education are required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Ability to complete CMS documentation expectations in NetHealth EMR.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures.
• Demonstrate sound judgment in evaluation, planning, and follow-up of resident therapy programs.
• Conduct screenings to determine need for intervention/treatment.
• Develop short and long-term treatment goals and plans.
• Interpret and communicate evaluation findings to residents and families.
• Complete CMS-compliant documentation in NetHealth EMR.
