Qualifications

• Active PT license in state(s) of practice.
• Bachelor’s degree in Physical Therapy from an accredited program.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide PT services per standards of practice and departmental procedures.
• Evaluate residents and develop treatment plans with short/long-term goals.
• Document evaluation, treatment, and outcomes; ensure CMS-compliant records.
• Communicate findings to residents, families, and the interdisciplinary team.
• Collaborate with the team to ensure quality resident care and productivity.
