Job summary

The Center at Tucson is actively seeking licensed Physical Therapists to direct and deliver therapy services at our skilled nursing facility in Tucson, AZ. The role emphasizes comprehensive patient assessment, collaborative care planning and physician-driven therapy plans to maximize patient function.

Qualified candidates hold a current PT license, FPCC, CPR certification and are prepared for background checks.

Qualifications

• Must possess a current and valid Physical Therapy license.
• Must possess FPCC clearance.
• CPR certification required.
• Pre-employment background check and drug test.
• 1 year of experience in a skilled nursing facility preferred.

Responsibilities

• Meet with administration, medical and nursing staff to plan therapy services with patients and families.
• Assist in developing, implementing and coordinating patient care plans.
• Explain procedures to patients and prepare equipment, reassuring patients during therapy.
• Determine proper equipment use and body positioning, adjusting as needed for therapy.
• Recommend modifications to the therapy program based on progress evaluations.
• Ensure therapy notes are informative and compliant with company policies.
