Job summary

New-Vista-Nursing-and-Rehabilitation-Cente is seeking a licensed Physical Therapist for a full-time role in a sub-acute/long-term care setting. You will evaluate patients, develop plans of care, and administer PT modalities as prescribed by a physician.

Responsibilities include directing treatment, supervising staff, documenting progress, and ensuring infection control with PPE. A minimum of 2 years SNF experience is preferred and NJ PT license is required.

Qualifications

• New Jersey Physical Therapy License (Required).
• At least 2 years of experience in Skilled Nursing setting.

Responsibilities

• Evaluate, plan, direct, and administer physical therapy modalities as prescribed by a licensed physician.
• Administer treatments and physical agents to restore function and prevent disability after injury or illness.
• Assist patients to reach their maximum potential within their capabilities.
• Supervise, coordinate, and delegate tasks to the supportive staff.
• Document patient changes accurately and on time; maintain infection control and PPE in patient areas.
