Job summary

Stratford Manor Rehabilitation & Care Center is seeking a licensed Physical Therapist to join our Rehabilitation/Therapy team. The role focuses on evaluating patients, developing treatment plans, and delivering therapy per prescriptions in a long-term care setting.

Ideal candidates will have a New Jersey license, experience in skilled nursing facilities, and strong communication and organizational skills to coordinate care and documentation in our electronic records system.

Qualifications

• New Jersey license required.
• Background/experience in long term care (Skilled Nursing Facility).
• Strong organizational, interpersonal and communication skills.
• Ability to manage patients with different personalities and levels.
• Creative problem solving skills.

Responsibilities

• Collaborate with therapy teams and Rehab Department.
• Complete evaluations and documentation.

• Offer treatments to improve patient abilities and challenges.

Local Skilled Nursing Facility Now seeking a Physical Therapist

Responsibilities/duties:

• Work collaboratively with the therapy teams and Rehab Department
• Complete evaluations and all necessary documentation
• Provide recommendations supporting communications/assessments
• Provide therapy as outlined in treatment plans and prescriptions
• Offer treatments that help improve patient’s inabilities and/or challenges
• Independently manage workflow which includes allotting time for treatment documentation of individual and group treatments and notes in the electronic records system
• Familiar with PDPM
• Align therapy treatments and plans with understanding of Medicaid and Medicare billing

Qualifications:

• New Jersey License
• Background/experience in long term care (specifically a Skilled Nursing Facility)
• Strong organizational, interpersonal and communication skills
• Ability to manage patients with different types of personalities and ability levels
• Creative problem solving skills
