Job summary

Shore Pointe Care Center in New Jersey seeks a dedicated Full-Time Physical Therapist to join our skilled nursing facility team. You will evaluate and treat residents in subacute and long-term care settings, developing personalized plans to improve mobility and independence.

Qualifications

• Active Physical Therapist license in New Jersey (or eligible)
• New grads welcome with mentorship and training
• Experience in SNF, long-term care, or geriatrics is a plus
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of an interdisciplinary team
• Compassionate, positive attitude focused on residents' well-being

Responsibilities

• Evaluate and treat residents in subacute and long term care with orthopedic, neurological, and post-surgical conditions
• Develop personalized treatment plans to improve strength, balance, gait and mobility
• Collaborate with physicians, nurses, OTs and SLPs
• Document patient progress accurately in EMR
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a team environment
• Supervise PTAs as needed
• Report to the Director of Rehabilitation

About the Role

We are seeking a passionate and dedicated Full-Time Physical Therapist to join our dynamic rehabilitation team in our skilled nursing facility in New Jersey. If you love helping seniors regain their mobility, independence, and quality of life, this is the perfect opportunity for you!

What You’ll Do

• Evaluate and treat residents in subacute and long term care setting with a variety of orthopedic, neurological, and post-surgical conditions
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise Physical Therapist Assistants (PTAs) as needed
• Report directly to the Director of Rehabilitation

What We’re Looking For

• Active Physical Therapist license in the state of New Jersey (or eligible to obtain)
