Job summary

Rose Mountain Care Center is seeking a dedicated Full Time Physical Therapist to join our rehabilitation team in New Jersey. You will evaluate and treat residents in subacute and long-term care, developing personalized plans to improve mobility and independence.

Collaborating with physicians, nurses, OT, and SLP, you will document progress in the EMR and participate in care plan meetings. New grads are welcomed and mentored in a supportive facility focused on compassionate, resident-centered

Qualifications

• Active Physical Therapist license in New Jersey or eligibility.
• New grads welcome; mentorship and training provided.
• Experience in SNF, long-term care, or geriatrics is a plus.
• Strong communication and interpersonal abilities.
• Ability to work effectively within an interdisciplinary team.
• Compassionate, patient-centered approach to care.

Responsibilities

• Evaluate and treat residents in subacute and long-term care with orthopedic, neurologic, and post-surgical conditions.
• Develop personalized treatment plans to improve strength, balance, gait, and mobility.
• Collaborate with physicians, nurses, OT, and SLP to coordinate care.
• Document progress accurately in the EMR system.
• Participate in care plan meetings and family conferences.
• Provide compassionate care in a team environment.
• Supervise PTAs as needed; report to Director of Rehabilitation.

We are seeking a passionate and dedicated Full Time Physical Therapist to join our dynamic rehabilitation team in our skilled nursing facility in New Jersey. If you love helping seniors regain their mobility, independence, and quality of life, this is the perfect opportunity for you!

What You’ll Do

• Evaluate and treat residents in subacute and long term care setting with a variety of orthopedic, neurological, and post-surgical conditions
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise Physical Therapist Assistants (PTAs) as needed
• Report directly to the Director of Rehabilitation

What We’re Looking For

• Active Physical Therapist license in the state of New Jersey (or eligible to obtain)
