Job summary

Domnovate is seeking a Permanent Physical Therapist in Pembroke, North Carolina, to apply clinical judgment and patient-centered care. The role is an on-site, direct-hire position with a standard 40-hour week, Monday through Friday, 8:00 AM to 4:30 PM, at an 84-bed skilled nursing facility focused on short-term rehab and long-term care.

Responsibilities include evaluating functional needs, delivering therapy, reassessing progress, and coordinating with the care team.

Qualifications

• NC Physical Therapy licensure or eligibility required.
• Accredited education and clinical qualifications for physical therapy practice.
• Strong clinical reasoning and patient-centered approach.

Responsibilities

• Evaluate functional needs and mobility limitations; set individualized goals.
• Deliver therapeutic interventions; reassess progress; adapt plan of care.
• Use exercise, gait, balance, strength and functional mobility strategies to connect treatment with daily activities.
• Explain techniques clearly; involve patients and caregivers in care.
• Collaborate with rehab and care team; document evaluations, interventions and progress.
