Qualifications

• Active Physical Therapist license in the state of practice (New Jersey).
• Current BLS/CPR certification preferred or required by facility policy.
• DPT degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehab, SNF, home health, or related settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and with an interdisciplinary team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, and pain management.
• Monitor progress, modify plans, and document evaluations and outcomes accurately.
• Educate patients and families on plans, home programs, injury prevention, and discharge info.
• Collaborate with physicians and healthcare teams to coordinate care and outcomes.
• Maintain timely clinical documentation per policy and regulatory requirements.
• Ensure compliance with patient safety, infection prevention, and quality standards.
• Treat patients across assigned settings and age groups as required.
