Talented Medical Solutions in New Hampshire is seeking a compassionate Physical Therapist (PT) to assess patients, design individualized rehabilitation plans, and guide interventions across acute care and rehabilitation settings.

The role emphasizes collaboration with physicians and nurses to restore mobility, reduce pain, and improve functional independence, with emphasis on patient education and accurate clinical documentation.

Formación

• Active Physical Therapist license in NH.
• BLS/CPR certification as required or per facility policy.
• Doctor of Physical Therapy (DPT) degree.
• Experience in acute care, outpatient, inpatient rehab, SNF, home health, or related settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and with an interdisciplinary team.

Responsabilidades

• Evaluate patients and develop individualized treatment plans.
• Provide interventions to improve mobility, strength, balance, and function.
• Monitor progress and adjust plans; document evaluations and outcomes.
• Educate patients and families on plans, home programs, and discharge.
• Collaborate with physicians and care teams to coordinate rehabilitation.
• Maintain accurate clinical documentation and ensure compliance with policies.
• Treat patients across assigned settings and age groups.
• Perform other duties as assigned.
