Job summary

PACS is seeking a Staff Physical Therapist in Montana to evaluate and treat residents, develop individualized plans, and educate families. You will supervise PT assistants, coordinate with physicians and nursing staff, and participate in care conferences to optimize rehabilitation outcomes.

Qualifications

• Bachelor’s degree in Physical Therapy (Master/Doctorate preferred).
• Licensed Physical Therapist in the state; maintain license with CE as required.
• Proficient with electronic medical records and rehab documentation.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential per physician orders.
• Develop, implement, and update individualized treatment plans to improve mobility.
• Supervise PT Assistants in resident care and related activities.
• Communicate progress and discharge planning with physicians, staff, families.
• Participate in care conferences and rehabilitation meetings as needed.
• Provide in-services on training programs for staff.
• Maintain timely, accurate documentation per regulations.
• Maintain safe treatment environment and infection control procedures.
• Educate residents and families on home exercise and fall prevention.
• Recommend and facilitate ordering of durable medical equipment.
