Job summary

Trinity Healthcare Talent Services seeks a skilled Physical Therapist to join a 84-bed Skilled Nursing Facility in North Carolina. You will assess patients, develop individualized plans, and coordinate care to improve mobility and reduce pain.

You will document progress, educate families, and collaborate with physicians and nurses while adhering to standards. A state license and 1–2 years SNF experience are preferred.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current state license to practice as a Physical Therapist.
• 1–2 years of experience in a skilled nursing facility or similar setting preferred.
• Strong knowledge of physical therapy principles, techniques, and modalities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate and patient-centered approach to care.
• Basic computer skills and proficiency in electronic medical records.

Responsibilities

• Evaluate patients' physical abilities, limitations, and medical history to determine the most effective treatment plan.
• Develop and implement individualized treatment plans that include therapeutic exercises, manual therapy, and other modalities.
• Monitor and document patients' progress and adjust treatment plans as needed.
• Educate patients and their families on exercises and activities to promote recovery and prevent future injuries.
• Collaborate with other healthcare professionals, such as doctors and nurses, to ensure comprehensive and coordinated care for patients.
• Maintain accurate and up-to-date patient records and documentation.
• Adhere to all legal and ethical standards and guidelines in the provision of physical therapy services.
