Trinity Healthcare Talent Services, LLC is seeking a skilled Physical Therapist to join a 74-bed skilled nursing facility team in Maine. The role focuses on post-hospital rehabilitation, long-term care, and respite services, aiming to help residents achieve maximum independence.

Responsibilities include evaluating patients, developing and adjusting treatment plans, and coordinating with families and other healthcare professionals. A valid state license and 1 year SNF experience are required.

Formación

• Valid state Physical Therapist license.
• Minimum 1 year of experience in a skilled nursing facility.
• Strong knowledge of rehabilitation techniques and modalities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and patient-focused approach to care.

Responsabilidades

• Evaluate patients' physical abilities and develop individualized treatment plans.
• Improve mobility, strength, balance, and coordination through therapy.
• Use therapeutic exercises, manual therapy, and other modalities to regain function and reduce pain.
• Monitor and document patient progress and adjust plans as needed.
• Coordinate care with patients, families, and other healthcare professionals.
• Educate patients and families on continuing rehabilitation at home.
• Supervise and delegate tasks to physical therapy assistants and support staff.
