Holy Cross Hospital Jordan Valley seeks a licensed Physical Therapist to provide a continuum of care for patients with neuromuscular and orthopedic conditions. You will guide mobility training, strengthening, and manual therapies, applying evidence-based methods while aligning with our mission and values.

You will evaluate patients, establish treatment plans with measurable goals, and collaborate with families and the healthcare team to optimize recovery and discharge planning.

Qualifications

• Bachelor's degree in Physical Therapy or equivalent.
• Licensed PT in the state of employment or a participating state under the National Licensure Compact.
• Basic Life Support certification from the American Heart Association.

Responsabilités

• Evaluate patients referred to physical therapy and develop plans within department standards.
• Create treatment plans with measurable goals and timelines in collaboration with patient and care team.
• Select and implement therapeutic activities to meet plan goals and track outcomes.
• Document evaluations, progress notes, and treatment records per standards.
• Reassess plans and adjust based on patient function and discharge plans.
• Communicate with patients, families, staff, and physicians while maintaining confidentiality.
