Job summary

Holy Cross Hospital Jordan Valley is seeking a licensed Physical Therapist to provide comprehensive rehabilitation services to patients with neuromuscular and orthopedic conditions. You will perform mobility training, strengthening, modalities, manual therapy, and neuromuscular re-education to restore function.

You will evaluate patients, design and adjust treatment plans, document outcomes, and collaborate with the care team while upholding core values and patient safety standards.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• PT license or eligibility in NLC is required.
• BLS certification from the American Heart Association is required.

Responsibilities

• Evaluates patients referred to Physical Therapy and creates initial evaluation within department standards.
• Develops and implements treatment plans with measurable goals and timelines.
• Selects therapy activities to meet patient goals and reassesses plans as needed.
• Documents all patient care activities per practice standards and payer requirements.
• Communicates with patients, families, and healthcare team to ensure coordinated care.
