Job summary

Simpsonville Post Acute in Simpsonville, SC is seeking a licensed Physical Therapist Assistant (PTA) to provide high-quality, patient-centered therapy under the direction of a Physical Therapist. This PRN position focuses on helping residents improve mobility and independence, with duties including documentation, care planning support, and collaboration with the interdisciplinary team.

Experience in skilled nursing or rehab is preferred.

Qualifications

• SC PTA license in good standing required.
• Graduate of accredited PTA program.
• Experience in skilled nursing or rehab preferred.
• Strong documentation and communication skills.

Responsibilities

• Provide therapy treatments as directed by the Physical Therapist.
• Follow physician treatment plans and care goals.
• Document daily treatments and weekly progress notes accurately.
• Assist with rehabilitation and care planning meetings.
• Communicate patient progress with the interdisciplinary team.
• Assist with discharge planning and caregiver education.
• Maintain clean, safe therapy areas and equipment.
• Support training of restorative aides and nursing staff.
• Ensure compliance with state practice act and facility policies.
