Job summary

Amedisys is seeking a Physical Therapist for Per Diem Days in adult home health visits. You will help patients stay safe and independent at home, providing PT interventions and regular reexaminations. The role includes discharge planning and other duties as assigned.

Minimum hourly pay ranges from $54 to $64, with benefits, PTO and wellness programs available. You will join a national home health leader committed to care at home.

Qualifications

• Licensed PT in the state you will work in.
• Graduation from a CAPTE-accredited PT program.
• One year of home health physical therapy experience is preferred.
• If <1 year of experience, mentoring/ residency may be required.
• Current CPR certification is required.

Responsibilities

• Help patients improve movement, activity and function.
• Provide interventions and conduct reexaminations every 30 days at minimum.
• Perform discharge planning and other duties as assigned.

Attractive Pay

Responsibilities

• Responsible for helping patients maintain, restore and improve movement, activity and function.
• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
