Job summary

Amedisys is seeking a Physical Therapist to join our home health team in a per diem daytime role. You will help patients maintain and improve movement and function, delivering interventions and performing periodic reexaminations, with discharge planning as needed.

Minimum hourly pay is $55 and can go up to $65, with full benefits including PPO or HSA plans, paid time off, wellness rewards, mental health support, 401(k) with match, and family support programs.

Qualifications

• Current license to practice physical therapy in the state where assigned.
• Graduate from a CAPTE-approved physical therapy program.
• One year of related physical therapy experience for home health care applicants.
• If less than one year experience, mentoring approval from regional clinical leaders is required and/or participation in residency program.
• Current CPR certification.

Responsibilities

• Provide interventions, conduct reexaminations (every 30 days, at minimum), adjust interventions as needed, and perform discharge planning.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
