Elara Caring in Owosso, MI is seeking a Physical Therapist for Home Health (PRN) with flexible self-scheduling to reach patients in Owosso, Ovid, Perry, Bath, Laingsburg, Byron, New Lothrop and surrounding communities.

You will evaluate patients, develop and implement individualized home health plans, provide therapy in patients' homes, monitor progress, document thoroughly, and collaborate with physicians and the care team.

Qualifikationen

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver’s license or reliable transportation to travel between patient homes and locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds.

As a Physical Therapist , you’ll contribute to our success in the following ways:

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
