Job summary

Elara Caring is hiring a Physical Therapist to deliver home-based patient care across Indiana. You will evaluate patients, develop individualized plans, and provide hands-on therapy in patients’ homes while coordinating with physicians and the care team.

The role requires an APTA-approved program graduate with an active PT license, at least 1 year in a healthcare setting, and the ability to travel between homes. Comprehensive benefits available.

Qualifications

• Graduate of an APTA approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or reliable transportation to travel between patient homes.
• Ability to frequently lift, push, pull, and move up to 50 pounds.

Responsibilities

• Evaluate patients and develop individualized treatment plans.

What is Required?

• Minimum one (1) year of experience in a healthcare facility

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
