Job summary

Elara Caring is seeking a Physical Therapist to deliver compassionate home health services across Owosso, Ovid, Perry, Bath and surrounding areas in Michigan. The PRN role offers flexible self-scheduling to fit your life while delivering high-quality care in patients' homes.

You will evaluate patients, develop individualized treatment plans, monitor progress, document clinically, and collaborate with physicians and the care team.

Qualifications

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess reliable transportation to travel between patient homes.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.

As a Physical Therapist, you’ll contribute to our success in the following ways:

• Evaluate patients and develop individualized treatment plans.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
