Job summary

Terraces of Lake Worth Care Center in Lake Worth Beach, FL is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team as a Per Diem clinician. You will conduct evaluations, develop individualized plans, and support patients across sub-acute and skilled nursing settings with compassion and evidence-based care.

You will collaborate with nursing, occupational, and speech therapy colleagues, educate patients and families, maintain accurate records, and contribute to quality

• Implement and modify treatment plans to address patient needs and goals, ensuring effective progress and recovery

• Maintain accurate and timely records of patient progress and treatment plans
