Job summary

Terraces of Lake Worth Care Center in Lake Worth Beach, Florida, is seeking a dedicated Physical Therapist (PT) to join as a Per Diem provider. You will evaluate patients, design and adjust treatment plans, and collaborate with nurses, OT, and speech therapy to support rehab goals.

You will educate patients and families, maintain detailed progress notes, and contribute to quality improvement initiatives in a warm, patient-centered environment.

Qualifications

• Conduct PT evaluations and develop individualized treatment plans for sub-acute and skilled nursing patients.

Responsibilities

• Conduct PT evaluations and develop individualized treatment plans for patients in our sub-acute and skilled nursing care settings
• Implement and modify treatment plans to address patient needs and goals
• Collaborate with nursing, occupational therapy, and speech therapy for comprehensive care
• Provide education and support to patients, families, and caregivers on physical therapy techniques and strategies
• Maintain accurate, timely records of patient progress and plans
• Participate in quality improvement and develop best PT practices
