Job summary

Terraces of Lake Worth Care Center is seeking an Occupational Therapist (Per Diem) to assess residents, create individualized treatment plans, and deliver therapeutic interventions. The role emphasizes improving independence and quality of life in a compassionate care setting.

You will collaborate with a multidisciplinary team, document progress, and participate in case reviews to support resident goals and discharge planning.

Qualifications

• Experience conducting occupational therapy assessments and evaluations.
• Ability to develop individualized, goal-oriented treatment plans.
• Proficiency in implementing OT interventions and activities.

Responsibilities

• Conduct thorough assessments of residents' physical and cognitive abilities
• Develop and implement individualized treatment plans to address residents' goals and needs
• Provide occupational therapy interventions, including ROM, strengthening, stretching, cognition and memory exercises
• Collaborate with other healthcare professionals to ensure comprehensive care
• Maintain accurate records of treatment plans, progress notes, and discharge summaries
• Participate in interdisciplinary team meetings to discuss resident progress and needs

About the Role

As an Occupational Therapist (Per Diem) at Terraces of Lake Worth Care Center, you will have the opportunity to work with a diverse range of residents, assessing their physical and cognitive abilities, and developing individualized treatment plans to enhance their overall well-being. Responsibilities
