Job summary

Concept Rehab, Inc. is seeking a part-time Physical Therapist to evaluate and treat residents in a skilled nursing facility in Adrian, MI. You will develop individualized plans to improve mobility and function, and collaborate with physicians and the rehab team to deliver quality care.

Qualifications

• Current Michigan Physical Therapist license or license eligible.
• Graduate of an accredited Physical Therapy program.
• Strong communication and clinical skills.
• Experience in skilled nursing, post-acute rehabilitation, long-term care, or geriatrics preferred; new graduates welcome.

Responsibilities

• Evaluate and treat residents in a skilled nursing facility.
• Develop individualized treatment plans to improve mobility, balance, strength, and overall function.
• Collaborate with physicians, nursing staff, families, and therapy team members.
• Complete timely electronic documentation.
• Supervise Physical Therapist Assistants (PTAs) as appropriate.
• Deliver compassionate, evidence-based patient care.
