Concept Rehab, Inc. in Adrian, MI is seeking a part-time Physical Therapist to join our skilled nursing facility team. You will evaluate and treat residents, develop individualized plans, and collaborate with physicians, nursing staff, and families.

Qualifications

• Current Michigan Physical Therapist license or license eligible.
• Graduate of an accredited Physical Therapy program.
• Strong communication and clinical skills.
• Experience in skilled nursing, post-acute rehabilitation, long-term care, or geriatrics preferred; new graduates welcome.

Responsibilities

• Evaluate and treat residents in a skilled nursing facility.
• Develop individualized treatment plans to improve mobility, balance, strength, and overall function.
• Collaborate with physicians, nursing staff, families, and therapy team members.
• Complete timely electronic documentation.
• Supervise PTAs as appropriate.
• Deliver compassionate, evidence-based patient care.
