Schedule: 4 - 10's or 5 - 8's. Mixed Setting: 90% Outpatient - 10% Inpatient Coverage.

What You’ll Do

• Evaluate patients and develop individualized treatment plans based on physician referrals

• Collaborate with physicians, nurses, case management, and therapy staff to optimize patient outcomes

• Participate in care planning, team meetings, and quality improvement initiatives

What We’re Looking For
