Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide evidence-based physical therapy interventions to restore mobility, strength, balance, and functional independence.
• Treat a variety of diagnoses including orthopedic, neurological, and post-surgical conditions.
• Educate patients and families on home exercise programs, recovery expectations, and injury prevention.
• Collaborate with physicians, nurses, case managers, and therapy staff to coordinate care plans.
• Maintain accurate EMR documentation including evaluations, progress notes, and discharge summaries.
• Participate in care planning meetings and quality improvement initiatives.
