The role involves assessing, developing, and implementing individualized treatment plans to improve mobility, strength, and overall well‑being across outpatient clinics, inpatient hospitals, home health, and rehabilitation centers. You will perform evaluations, apply evidence‑based therapies, educate patients and families, coordinate with multidisciplinary teams, and document care in EHRs.

Qualifications

  • Valid licensure as Physical Therapist in the respective jurisdiction.
  • Experience in inpatient hospital, outpatient, or home health settings.
  • Strong knowledge of physiology, anatomy, and medical terminology.
  • Experience with geriatric, pediatric, neurology, orthopedics, or outpatient rehab is preferred.
  • Familiarity with EHR/EMR systems and clinical documentation standards.
  • Ability to develop comprehensive treatment plans from assessments.
  • Excellent patient education and communication skills.
  • Knowledge of discharge planning and collaboration with healthcare teams.
  • Experience across diverse care settings including inpatient, outpatient, home care.

Job description

We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. In this role, you will be responsible for assessing, developing, and implementing individualized treatment plans to improve patients' mobility, strength, and overall well-being. The ideal candidate will possess strong clinical expertise across various settings, including outpatient clinics, inpatient hospitals, home health, and rehabilitation centers. Your efforts will directly contribute to enhancing patient recovery outcomes through evidence-based physiotherapy practices and compassionate care.

Duties
  • Conduct comprehensive patient evaluations to determine functional limitations and develop personalized treatment plans aligned with clinical standards.
  • Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs tailored to patient needs.
  • Collaborate with multidisciplinary healthcare professionals to coordinate care plans, discharge planning, and ensure continuity of care.
  • Monitor patient progress regularly, adjusting treatment strategies to optimize recovery and functional gains.
  • Maintain accurate clinical documentation in accordance with healthcare regulations and electronic health records (EHR) standards.
  • Educate patients and their families on physiotherapy techniques, injury prevention, and home exercise programs.
  • Assist in the development of care plans for diverse populations including geriatric patients, pediatrics, neurology cases, orthopedics, and outpatient rehabilitation.
  • Support discharge planning by evaluating patient readiness for transition to home or community care settings.
  • Ensure compliance with medical terminology standards and physiotherapy protocols during all patient interactions.
Requirements
  • Valid licensure or certification as a licensed Physical Therapist in the respective jurisdiction.
  • Proven experience practicing in a physical therapy role within inpatient hospital settings, outpatient clinics, or home health environments.
  • Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice.
  • Experience working with geriatric patients, pediatrics, neurology cases, orthopedics, or outpatient rehabilitation centers is preferred.
  • Familiarity with clinical documentation standards and electronic health records (EHR) or electronic medical records (EMR) systems.
  • Ability to develop comprehensive treatment plans based on patient assessments and clinical data.
  • Excellent client communication skills to effectively educate and motivate patients throughout their recovery journey.
  • Knowledge of discharge planning processes and working collaboratively with healthcare teams for optimal patient outcomes.
  • Demonstrated ability to work in diverse settings such as inpatient hospital units, outpatient clinics, home & community care environments.
