BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per diem opportunity to perform home health visits for adult and geriatric clients in the Greenville, Pickens, Spartanburg, & Anderson Counties area.

As a home care Physical Therapist, you will be a key member of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Implement wound prevention and repair interventions using evidence-based dressings and topical agents.
4. Develop and revise individualized care plans with manual exercises, gait training, and balance interventions.
5. Assess and update care plans regularly, collaborating with nursing staff as needed.
6. Educate clients and families on rehabilitative activities to promote health, safety, and independence.
7. Document observations and interventions accurately using electronic tablets.

Qualifications:

1. Current SC Physical Therapist (PT) license.
2. Graduation from an American Physical Therapy Association-approved program.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with electronic medical records preferred.
