Physical Therapist (PT) - Home Health Case Manager

This position is located near South City, focusing on ZIP codes 60616, 60608, and 60623. The hours are days with rotation to weekends and one summer/winter holiday. It is eligible for a sign-on bonus of up to $10,000, contingent on employment at the time of payout. Internal team members are not eligible.
Major Responsibilities
- Assuring and Improving Quality of Care: Facilitates quality, cost-effective care following Advocate's MVP of Stewardship. Utilizes resources, adheres to evidence-based standards, participates in quality initiatives, promotes safety, and maintains thorough documentation.
- Care and Service Coordination: Works effectively as part of the care team, participates in meetings and educational activities, communicates professionally, and seeks ongoing development.
- Leadership and Professional Development: Demonstrates clinical decision-making, recognizes clinical changes, prioritizes patient care, and participates in interdisciplinary case conferences.
- Patient Relationship: Builds restorative relationships, exceeds patient expectations, promotes self-care, and uses adaptive communication styles.
- Business Development and Growth: Enhances productivity, adheres to policies, and monitors resource utilization.
- Patient Education: Meets learning needs, assesses readiness, and provides tailored educational resources.
Licensure, Registration, and Certification
- Illinois Physical Therapy License.
- Basic Life Support (BLS) certification from the American Heart Association.
- Valid driver’s license.
Education and Experience
- Doctorate in Physical Therapy from an accredited institution or equivalent, with ongoing continuing education.
- At least 1 year of clinical experience or strong clinical affiliation experience.
Knowledge, Skills & Abilities
- Computer proficiency (Microsoft Word, email), ability to learn documentation systems, strong communication and documentation skills, interpersonal skills, time management, and proficiency with home therapy equipment.
Physical Requirements and Working Conditions
- Extensive travel in various weather conditions, working weekends and holidays, attending meetings, and working in patient homes or facilities. Must be able to lift at least 25 pounds, with some situations requiring lifting up to 35 pounds, using patient handling equipment when necessary.
