Job Summary:

Pleasant Home Health is seeking a compassionate and skilledPer Diem Physical Therapist (PT)to provide high-quality, patient-centered care in the home setting. The PT will assess, develop, and implement individualized treatment plans to help patients improve mobility, reduce pain, and restore function.

Responsibilities:

• Conduct comprehensive evaluations of patients' physical abilities and medical histories.
• Develop and implement personalized treatment plans to improve strength, balance, coordination, and mobility.
• Provide therapeutic exercises, manual therapy, and modalities as needed.
• Educate patients and caregivers on home exercise programs and injury prevention.
• Document patient progress and adjust treatment plans accordingly.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.
• Maintain compliance with all regulatory and company policies.

Requirements:

• Graduate of an accredited Physical Therapy program.
• Minimum of 1 year of experience in home health or related setting (preferred).
• Strong clinical and interpersonal skills.
• Reliable transportation and willingness to travel to patient homes.
• CPR certification (mandatory).
• Competitive per-visit/per-hour rate.
